Case Management & Self-Sufficiency

  • Conduct comprehensive intake assessments to identify immediate needs and barriers to integration.
  • specific “SMART” goals (Specific, Measurable, Achievable, Relevant, Time-bound) in partnership with clients to map their path to independence.
  • Administer direct financial assistance and facilitate applications for public benefits (CalFresh, Medi-Cal, etc.).
  • Connect clients with vital community resources, including medical providers, mental health services, and vocational training.

Compliance & Reporting

  • Maintain rigorous documentation of all client interactions in the ETO (Efforts to Outcomes) database.
  • Ensure all service delivery meets the strict eligibility and reporting requirements of federal resettlement programs.
  • Monitor client progress continuously, adjusting service plans as challenges arise.

  • Minimum of 2 years of professional experience in human services.

  • Digital Literacy: Ability to navigate complex case management databases (IRIS, ETO) and proficiency in Microsoft Office.
  • Organization: A self-starter with the ability to prioritize conflicting duties in a fast-paced environment.
  • Diplomacy: Strong interpersonal skills with the ability to build trust across cultural lines.

Mandatory Requirements

  • Valid driver’s license.
  • Access to a reliable, insured personal vehicle.
  • Willingness to work occasional evenings or weekends to accommodate client arrivals or emergencies.
